• Age proof certificate, only HSLC or CBSC Admit Card / Birth Certificate issued by concerned. District Health Authorities will be accepted.
• Graduate Certificate or equivalent Certificate for proof of educational qualification.
• Proficiency Computer skill for 3 (Three) months course for Junior Assistant.
• 3 (Three) recent self attested passport size photographs.
• Permanent / Temporary Address Proof.
• Mobile Number with whatsapp facilities.
• E-mail ID, if any

All documents must be produced both in Original and Self attested copies.

The applicant must reach the venue by 10:00 AM for enrollment of their names etc.
